My grandma is in a nursing home and wants to come home, we want her home. they wont let her come home.......?

Well I took care of my grandma for 4 months after we found out she is in critical condition. The nurse through hospice reccomended respite for a week. Gma ended up being SENT to a nursing home? And shes been there a month. I keep trying to get her home but to no avail, my dad too. He has power of attorney. What to do?
